Download here. Forms must be completely filled,
signed and dated. One form per applicant. Do not leave any areas blank but instead write "N/A".

One color 2x2 photo taken by a professional passport service, Walgreen, CVS, Kinkos, etc.

Invitation Letter
An invitation letter from a Chinese company or a business letter from your own company
on your company letterhead. See a sample business letter
If you are applying through our Chicago office, you need to submit both letters: invitation from China and business letter from your own company. Please call us if you have any questions.
