Child Passport Instructions

 

1. Gather and prepare the following documents:

A) Completed US passport application

DS-11 Form: Click here to complete and create your child's U.S. passport application form online. After you’ve provided all required information, click the “Create Form” button and your application form will appear completed on your screen. Print out the form, but DO NOT SIGN it until instructed to do so by the local Acceptance Agent!

B) Proof of Relationship

Please supply one of the following:

  • Minor’s certified U.S. birth certificate with both parents’ names

  • Minor’s certified Foreign Birth Certificate with both parents’ names

  • Minor’s Report of Birth Abroad with both parents’ names

  • Adoption Decree with adopting parents’ names

  • Court Order establishing custody

  • Court Order establishing guardianship

C) Proof of Identity
  • Previously issued, undamaged U.S. passport

  • Valid Driver’s License

  • Current Government ID (city, state or federal)

  • Current Military ID (military and dependents)

D) Passport Photos

Two identical 2×2 color photos with a white background; Photo must be taken at a professional passport photo service such as Walgreen, CVS or Kinko’s.

E) Letter of Authorization (for Minors)

Fill out and sign two copies of the Letter of Authorization.  The letter is included in the PDF packet generated after completing the online order. This document will authorize CVSC to submit and obtain a new passport on your behalf.

F) Proof of Departure

You must provide one of the following:

  • Copy of plane ticket

  • Computer generated travel itinerary issued by travel agent

Dates on travel itinerary must match those on Form DS-11 and the Authorization Letter.

G) US Government Fees
  • $140.00 in check or money order payable to US Department of State ($155.00 if you are ordering a passport book and a passport card.)

  • $25.00 execution fee payable to local Acceptance Agent.

a. Write applicant’s full name and date of birth on the check or money order.
b. Personal checks must show a printed address. Temporary or starter checks are not acceptable.
c. Make a copy of the check and include it in the package outside the sealed envelope.

2. Get Your Application Authorized

You need to have your new U.S passport application authorized by local government officials called Acceptance Agents (AA). Bring all of the documents listed in Step 1 to an Acceptance Agent at your local post office or court house, who will check your documents, witness your signature on the DS-11 form and seal some of the application materials in an envelope.

Application form (DS-11):

  • Two passport photos

  • Proof of citizenship

  • Proof of departure (optional)

  • Letter of authorization (one copy)

  • Payment to “U.S. Department of State”

The AA should write the applicant’s name, departure date, and date of birth on the outside of the sealed envelope. If two applications are sealed in the same envelope (normal), ensure that both applicants’ information appears.

For children 15 years and younger, both parents must appear with the child at the Acceptance Agent when obtaining the sealed envelope. If either parent cannot be present then a Written Statement of Consent form (DS-3053) must be provided to the AA. A copy of this form is included in this packet for your convenience.

Tell the AA that you are using a passport agent to expedite the US passport application. They must return the envelope to you. It is occasionally necessary to refer the AA to the Department of State’s “Hand Carrying of Executed Applications” policy, located on page 31 or 44 of their manual.

Once you have the sealed envelope, and the additional materials (as listed above), continue to Step 3 to receive your U.S. passport for minors. 

IMPORTANT

  • Do not open this sealed envelope or the application will be invalid

  • Send a copy of your travel itinerary and letter of authorization outside sealed envelope

  • The sealed envelope is valid for only five business days. After completing your order request, please forward sealed envelope to CVSC for immediate processing.

3. Send Your Application to CVSC

To receive the US passport, send above documents along with our Order Form to one of the following addresses (please send the material to our office nearest to you; but be rest assured that your application will be processed no matter which of our offices receives it) – For Rush or Express Service, you must send FedEx Priority Overnight or UPS A.M. Delivery only:
