| A) Completed US passport application | ||
|
DS-4085 Form: Click Here to complete and create your application form online. After you've provided all required information, click the "Create Form" button and your application form will appear completed on your screen for you to print out and sign.

| B) Your current U.S. passport | ||
| Your passport must be in good condition. The U.S. Department of State will not accept it if it is mutilated or damaged. Passport must have more than 1 year of validity remaining. | ||
| C) Letter of Authorization | ||
| Fill out and sign two copies of the Letter of Authorization. This letter will authorize CVSC to submit and obtain a new passport on your behalf. | ||
| D) Proof of Departure | ||
| You must provide one of the following: | ||
 Copy of plane ticket; Computer generated travel itinerary issued by travel agent; |
||
| Dates on travel itinerary must match those on Form DS-4085 and the Authorization Letter. | ||
| E) US Government Fees: | ||

or money order payable to "U.S. Department of State" a. Write applicant's full name and date of birth on the check or money order. b. Personal checks must show a printed address. Temporary or starter checks are not acceptable. |
